NICE NG143 Traffic Light · Febrile Seizures · Paediatric Sepsis

NICE NG143 (2021) — Fever in under 5s. The traffic light system stratifies risk of serious bacterial infection (SBI). Use clinical judgement alongside this tool. A child with any RED feature requires immediate referral regardless of other GREEN features.

ActivityResponds normally to social cues; content/smiles; stays awake or awakens quickly; normal cry or not crying
RespiratoryNormal respiratory rate; no recession; no nasal flaring; O₂ sat ≥95% in air
HydrationNormal skin and eyes; moist mucous membranes
OtherNo amber or red features
Management: Manage at home. Safety-net advice. No routine antibiotic. Paracetamol/ibuprofen for comfort. Return if amber/red features develop.
